Program objectives

The overall goal of the STREAM program is to help physicians and trainees to build skills to reduce stress, burnout and the effects of poor mental health. Specifically, we aim to:

  • Develop an equitable and inclusive skills-based curriculum for pediatric faculty and residents addressing four professional domains: Attending to mental health, professional resilience, engagement in systems improvement, and joy and meaning in work.
  • Implement the AFSP Mental Health interactive screening program (ISP) to allow participants to see their current state of mental health and to connect them to institutional resources if they would like.
  • Expand STREAM curriculum to include a total of 25 participating institutions by December of 2024

    Pillar objectives

    Mental Health Module:

    • Reflect on the foundational concepts that put physicians at risk for mental health conditions
    • Identify evidence-based methods that promote positive mental health by mitigating the negative aspects of the “medical culture”
    • Commit to periodic self-reflection on mental health as a vital sign
    • Develop and implement a mental health well-being plan and identify an accountability buddy or team
    • Discuss personalized, proactive strategies for optimizing mental health and protecting against suicide risk
    • Know when and how to approach a colleague you may be concerned about
    • Identify organizational strategies that can reduce the suicide risk among health workers, trainees, and employees

    Engagement Module:

    • Appreciate the relationship between physician engagement and overall wellness
    • Reflect on areas of personal importance related to systems issues that contribute to their own/their colleagues’ burnout
    • Identify explicit ways to become involved in systems-improvement efforts in their organizations Resilience Module:
    • Identify the 6 facets of the PERMAH Model of Well-being
    • Examine how the components of well-being appear at different times in your life
    • Be acquainted with scope and scientific underpinnings of mindfulness and meditation
    • Identify how to assess “mindfulness” for personal application
    • Understand role of Positive and Negative affect/emotion as normal processes within PERMAH

    Resilience Module:

    • Identify the 6 facets of the PERMAH Model of Well-being
    • Examine how the components of well-being appear at different times in your life
    • Be acquainted with scope and scientific underpinnings of mindfulness and meditation
    • Identify how to assess “mindfulness” for personal application
    • Understand role of Positive and Negative affect/emotion as normal processes within PERMAH

      Model of Well-Being:

      • Understand Flourishing Ratio as it applies to everyday living
      • Practice exercises designed to cultivate positive emotion
      • Integrate valued directions as a guide toward balance and meaning in the PERMAH Well-Being Model
      • Differentiate between Goals and Values
      • Understand role of barriers as guide toward values based living

      Joy & Meaning Module:

      • Understand the importance of promoting positive psychology, connecting to colleagues and purpose, and improving the clinical-learning environment
      • Utilize reflection with colleagues and re-discover joy and purpose in medicine, sense of calling, and purpose
      • Describe the power of sharing stories
